So I have been on 150mg of Test. Cyp 1x per week and 1500iu 1x per week of HCG for exactly 6 weeks starting tomorrow. I have my first 6 week checkup on Monday at 1pm and I know I haven't felt any better and if anything I have felt worse since starting TRT. I had a feeling something wasn't right and with the wealth of knowledge I have gained from reading on this site (thanks to all of you! :D) I knew I should get the basic tests done to see whats going on and to have results ready to make the most of my appointment (why not save a little money while I'm at it too considering what my Doc will charge me for the same tests) Anyways I took the tests about 48 hours after my weekly injections (9-10am) which I know is NOT ideal and levels will be much higher than when I should've taken the blood test (before my shots on injection day) Here are the results:

Total Testosterone: >1500 ng/dl Ref.: 348-1197

Free Testosterone: 46.9 Ref.: 9.3-26.5

TSH: 2.110 uIU/mL Ref.: 0.450 - 4.500

Estrodial, Sensitive: 77.1 pg/ml Ref.: 8.0 - 35.0

I have read so many conflicting things on this site and all over the internet about TRT and E2 levels in men. One thing I know for sure is i feel TERRIBLE and much worse than before I started TRT and I was feeling bad enough to get tested for low T etc. which came out to be Total: 160 Free T: 4 (about 10 weeks ago). I feel EXTREME fatigue everyday, even when I sleep for 8+ hours and take a 1-3 hour nap everyday, I had low libido to begin with but it is truly nonexistant now. Hard to get an erection (EVEN WITH CIALIS!) and the depression and anxiety I am having is miserable as well. Nipples seem very sensitive as well and it seems impossible to lose weight no matter how much I run/exercise or what/how little I eat. Could high e2 be causing this? Or is this a normal ratio or what not and just the way it's going to be for me? Sorry for such a long post, I just had to get all of this off of my chest. I love this site by the way, full of great information/solutions and great people. Thanks to everyone in advance.

Yes, 150mg a week of test cyp and 1500iu of hcg 1x per week (both on the same day). I have not been given or discussed an AI with my Doctor

I think the advice above to go twice per week is good, split that 150...which is too much at one-time, could explain that astronomical E2 reading. My opinion is the HCG of 1500iu is a ridiculously high dosage. most guys use 250iu EOD.

I am still incredibly new to all of this, but OF COURSE you feel horrible! Your numbers are all over the place, your hormones are out of whack! The high E2 is certainly to blame for a lot of it. Get into your doc, get everything corrected, and you'll feel better in no time :)

Don't get discouraged you got this! But you gotta be diligent about getting in to your doctor, and making sure you have the right help!

Most of the advise here is spot on.

Split your dose to 75 mg Monday AM and Thursday PM.

HCG can aroamataze just like Testosterone and the 1500 iu a week is a lot. Ask your Doc to go on 100 iu daily of HCG each morning...that's all you need and it's half what you are taking now.

Also, you will probably need an AI to bring you E2 down. Ask your Doc to go on .25 mg 24 hours after each injection per week for just 6 weeks and then get retested again.

Bet ya a bunch of beers your numbers will a lot better and you will feel a ton better as well.

Your T is high, and thus your estradiol is also. Your HCG dose is also high.

These factors can promote more water retention (higher BP), more sleep apnea, and make you tired and out of breath.

Try to go down to two injections a week of 50 mg T plus 250-500 IU HCG and let us know how you feel.

I would hold off on anastrazole since your E2 will come down once your T decreases.

All the above is noted and good advise. You can anticipate that the large spikes are due ..
1) Testing 2 days after medicating, which is at the very top peak level.
2) Injecting everything (as noted) at once, including that ridiculous amount of HCG.

All of that is no doubt contributing to the E2 matter as well, and some of it might be intratesticular aromatization, which can be problematic with too much HCG.

Here's my .02 for what it's worth, with what I would suggest ... I'd ramp back the testosterone to 100mg/wk total, dividing that into 2 injections of 50mg for the week. I'd cut back the HCG to 200iu or 250iu at most, injecting 3 times per week (say Mon, Wed, Fri).

As discussed already, you do need to review a AI, Anastrozole is usually a good choice, easy to manage. Even though your E2 is up there, and you probably want that down in the 20's, I wouldn't get carried away with mega dosing of that compound. Something in the lines of .75mg to 1mg at MOST is my suggestion, but review that with a qualified physician. If you can break that up 2 to 3 times per week as well, all the better. If someone tells you to take 2mg, 3mg, or more, word to the wise, don't do it. If it takes a month or two to get it in range, so be it, it should IMO not be a "rushed" protocol. If you crash your E2 you're in for a whole set of new problems, you don't want those!

If you have other labs (CBC's, metabolic, lipids, etc) please post them up?!?! The thyroid TSH is OK, but really that doesn't tell us anything about your actual thyroid hormone levels. Looking for Free T4, Free T3, Reverse T3 & Antibodies (unless you know you're clear of Autoimmune disorders).
