Hello, I am new to this site. For starters 27 year old male. Was diagnosed with low t almost 3 years ago. Been on trt since. My main question is I have a lot of big swings in my libido and mood. Especially my libido. My trt doctor that I see only does the standard e2 test. I wasn’t feel the greatest so last week went in for labs (all they check is your T, e2, PSA, SHGB, and hemocrit) anyways i informed them my sex drive was really low along with low libido. When my labs came back they said everything was fine. My testorone was 1050 and my e2 was 21. They told me to keep taking my anastrolze (.25 once a week) my question is, since they don’t do sensitive test am I getting an accurate result on my e2 readings? Because there are weeks where everything feels great. Obviously I don’t go in for labs then because I feel good. Should I be getting a sensitive test and is taking an AI even worth it. Because from what I have read yes you don’t want your e2 to high. But it seems like maybe 21 on a normal reading would be really low on a sensitive reading. Thanks for anyone who responds! Have a great day! Also my SHGB was 39.5, if I remember correctly. Sorry my doctor really seems to only focus on T and E as being a big factor in mood and sex drive

Also if anyone has some pointers I would be so greatful. Sorry I just ask my doctor questions like these and he just brushes them off. I get there isn’t an exact science behind it. I just want to know if I am missing something and maybe he is missing something.
 
LIbido as in your interest or desire to have sex, or libido as erection problems? Two different things. My first guess is with your SHBG @ 39 your E is too low @ 21. between the two tests you can generally assume that the sensitive test will read lower than the regular. Not always, but generally. I would stop the AI.

Ya you’re spot on along with everyone that commented. Sensitive E2 could definitely be even lower than 21. A loose rule of thumb is guys do well when they get their SHBG and E2 to be around the same. Even if this doesn’t hold true tho, I would say u would probably do better with an E2 between 25-40 based off of your SHBG. So say your sens E2 comes back at 15, u basically need double the amount of E2 that u currently have.

Regardless of the specifics, definitely seems like the solution is to let your E2 go up a little. I would personally cut the anastrozole in half and see how u feel on 0.125mg once a week for a couple weeks. If still no improvement, I would drop the AI all together.

What’s your free testosterone level? Because another option would be to keep AI the same and just raise your testosterone dosage a bit to get your free T up. I would personally try cutting down the AI and possibly dropping it first, but this could be an option if that ends up not improving things for you.

If not a lot, I recommend you get a few books.
Get a few good books and educate yourself now:
1. Get Nelsons book "Testosterone: A Man's Guide".
2. Get Lee Myer's book "Natural Versus Testosterone Therapy".
3. Get “Testosterone for life - Dr. Abraham Morgentaller “.
4. Get “The Definitive Testosterone Replacement Therapy MANual: How to OptimizeYour Testosterone For Lifelong Health And Happiness” by Jay Campbell.
These books will give you a much better understanding of TRT....

Well since my libido is down, my erection quality is down to. One thing I forgot to mention is my sensitivity down there is almost gone and it is next to impossible to finish at times. I assume that’s low E2. But from all the research I have down it seems low e2 and high e2 almost have identical symptoms. Of course though a reading of 21 wouldn’t be on the high end. That’s very interesting about they SHGB matching up with the e2. Is that a common thing? Thanks for the reply!

Here's my Estradiol, Sensitive 29.5 range 8.0 - 35.0 and SHBG 48.8 range 19.3 - 76.4. I feel great why would I want to match it?

This doctor is sabotashing you by not using the sensitive estrogen test, I'll bet your E2 is lower than stated. Low E2 is worse than high E2 for libido and erections and this doctor clearly doesn't know how this TRT game is done. Most men would have high E2 problem in the 1000 ranges. I would just skip the AI for a week or two, than restart at a lower dosage.

Taking the AI once weekly is a mistake, it means E2 will fluctuate during the week do to the short half life. I would recommend taking the AI twice weekly. I would also recommend two injection a week given your SHBG levels.

Your also stating to split the injections? Which I do, is 1050 considered high? I do my labs 3 days after my injection. And that was with an injection of 70mg. And yes at times I feel my doctor is just going along with stuff at times. It’s frustrating because when things are working properly it’s wonderful. No Viagra needed, a little foreplay with my wife and I am ready. When things aren’t going well. Viagra doesn’t work and all I end up with is a major headache. Luckily my wife is SUPER understanding of the issues. Honestly since we have had our daughter and with our work schedule sex is the last thing on her mind. I’m pretty sure if I never was the one to make the first moves for sex, we probably wouldn’t have sex for months if it was up to her haha. She even point blank told me “if I don’t care about your penis working and your sex drive, why do you?” I respond with “because i am a guy and it’s important at 27 to feel and function like a normal 27 year old.” Also my doctor told me that the sensitive test are “a waste of money and don’t really matter” I am staring to think he is wrong. That’s actually why I turned to this board. Because it just seems like my doctor wants his monthly payment send me my at home injections and if the numbers are good ‘according to them’ not bother him or the clinic. FML

Another thing about skipping a dose for a week or two. About 2-3 weeks ago (can’t remember the exact time) I did skip a dose of my AI. I did labs and my e2 came back at 30 (mind you this is the normal test as I have stated not the sensitive one) the PA at the clinic said my e2 was getting a little too high and I need to immediately get back on my AI. I just want to know, because from reading forum after forum it seems like the people on forums ‘this one included’ know wayyy more then the doctors. But what I am real curious about is if they are really pushing using the AI too much. Like the PA was legitimately worried when my e2 was 30

Hormone replacement requires that the necessary tests be run and doctors who take insurance play by a different set of rules by skipping necessary labs to save insurance companies money.

Perhaps your wife is experiencing a hormone deficiency as well, common after childbirth. Your women should be wanting to be close with her husband to maintain a healthy relationship, the lack of bonding will remove the closeness in the long run since oxytocin is the bonding hormone, the love hormone which is released after sex and without it love will fade in time.

I hear ya partially on that. The only reason it puts me at ease is because even though I am doing everything in my power to get it fixed (research, talking to people knowledgeable about it, possibly finding another doctor) I dont feel this immense pressure from my wife. Even though I feel it from myself. And I think you might be right about the post child Thing. Because man after we had a kid everything dropped immensely for her. She even has admitted since having our daughter she feels kind of depressed more at times. Also
