Hey guys, just wanted to introduce and get some info, advice and suggestions on how I can address my frustration for TRT. 43 male, overweight but working that. I'll give you the readers digest version of my plight:

Several years ago I struggled with no energy, tired, wiped out, etc and had my total T level checked and it was 341. I was referred to a Urologist who was a punk. He said my T level was just fine and all that was wrong with me was I was depressed and needed to exercise. He did order a total and free, which were 342 and 9.4, respectively in early 16. After this quack received the results he let me know that he wouldn't do anything for me and to get me some depression meds and start exercising more. I ended up seeing an alternative medicine doctor who put me on T Cypionate. Unfortunately it was all out of pocket to see him and it was 400-600 bucks every time and I just couldn't fit it in my finances. In addition, he was the closest around to me and he was an almost 2-hour drive away, one way.

Fast forward to now, I am so freaking exhausted every day that most days I have to come in from work and sleep for an hour before I can do anything. I'm in Executive Management and I have such brain fog some days that I can't hardly function in decision making. No sex drive, no libido whatsoever and erections are hard to come by (no pun intended). I have no endurance whatsoever, I can't hold out to do anything I finally got so fed up I went to my new PCP and demanded help. He referred me to knew Urologist who prescribed Androgel. The new urologist refuses to do shots because he says they create too many peaks and valleys. Plus, they make my Hemoglobin go way to high, and he's just not comfortable doing shots. He is willing to do the gel and pellets. My insurance is THROWING UP! It's been almost a month and I'm still not approved. The insurance wants another T level. My last one drawn was a month ago and it was TT 231, free 5.1. I have found a compounding pharmacy that will do a cream equal to Androgel for 60 bucks. I am willing to pay out of my pocket to get my life back.... How do I convince him to give me a scrip until they get this Androgel fiasco figured out?

I am one of the few here that do well on a daily compounded "T" cream from EmpowerRX.
Reasonable cost and I pay "out of pocket", and it is less than what my co-pay would be if I was on Androgel or similar!!!
You might want to call Defy and get hooked up with them.
Some of the current post indicate a bit of a wait to get hooked up, but from all reports it is worth it.
Verrrrrrrrrrrry knowledgeable in this area!!!!!

Cannot tell how much research you have done on TRT.
If not a lot, I recommend you get a few books.
Get a few good books and educate yourself now:
1. Get Nelsons book "Testosterone: A Man's Guide".
2. Get Lee Myer's book "Natural Versus Testosterone Therapy".
3. Get “Testosterone for life - Dr. Abraham Morgentaller “.
4. Get “The Definitive Testosterone Replacement Therapy MANual: How to OptimizeYour Testosterone For Lifelong Health And Happiness” by Jay Campbell.
These books will give you a much better understanding of TRT....

Welcome to the forum. It sounds like to me you need to drop this bozo of a doctor and if I were you I would consider Defy Medical. They can handle everything on a tele-med basis and you won't find another medical group that will do TRT any better. Dr. Saya, Medical Director with Defy Medical, regularly participates on this forum. I have spent the majority of my life in executive management and I am sure, like me, you do not have time to diddle around with those that can't or won't help you. Since you are a decision maker, make a decision and bolt over to Defy or someone else. Defy is proven, another PCP might just be what you already have. Lot's of guys on this forum use Defy and can attest to their knowledge and value.

My new PCP is the one who referred me to the new Urologist. I am just getting completely frustrated at the time it's taking to get Androgel. I have a compound pharmacy 20 min from me; I just need him to give me a prescription, which I don't know if he is pro compound. Can you advise me how this defy works? I have a current T report, both total and free.

I have a full gamut of blood work done; my thyroid was completely fine.

How did you convince your doctor to give you a script for the compounded cream? That's where I am at. I am supposed to call him this week and I want to be armed and ready.

Some people do well on Androgel, Testim, or creams and many do not. I tried Androgel and it was “okay” at best but it got to where the insurance companies did not want to cover it. Once I finally got approved the script would only be good for 6 months. Every time I tried to renew the script it took two to three months to finally get the insurance company to approve it. I got tired of the insurance companies controlling my health and well being. You are experiencing that now. Just wait til you go through that process for a few years. With Androgel you have to be very careful with it so you do not get it on you wife, children, grandchildren, etc....you can easily wash it off if you get the area you applied the gel to wet and if you sweat too soon after applying you can sweat it off. Lastly, you do not know how well your body will absorb the gel. I switched to injections and am so glad I did.

What was your protocol when you were on injections? How high did your hemoglobin climb and what sort of your discussion did you have with your doctor in response to its rise?

I was on Test. Cypionate 100 a week. My hemoglobin went to 16. It was considered drawing off a pint or so, but then he just expressed high concern that I wasn't tolerating the injections well based on that point.

I certainly hope you are successful in your quest for compounded cream; it's a rational approach to TRT. I only want to point out that the resistance you're encountering in regard to injections reveals that the doctors you're dealing with aren't adept at managing contemporary protocols. The challenges - rising Hgb, peaks/valleys - can be managed.

You deserve quality care, and I hope you find it. Good luck!

There are a lot of red flags here that suggest your dealing with incompetent doctors, if you were injecting 100mg all at once every week, there's your problem. Most guys find success injecting smaller multiple dose a week, this keeps hematocrit and hemoglobin under control and you feel better as a result, but if your working with an incompetent doctor he's not going to know what to do.

Hemoglobin at 16, I don't see a problem. This poor doctor is in way over his head, perhaps he should leave TRT to the hormone specialists.
