Estoy muy preocupado, llevo un año de médicos y no consigo aclarar mi problema.

Tras un periodo de estrés muy elevado y una disminución total de mi capacidad sexual decidí acudir al medico, tras varias visitas me mandaron inicialmente a un internista, después visite un urólogo y ahora me ha visto un endocrino. Para resumir doy los datos que entiendo que son la base del problema o al menos de mi preocupación.

En el primer análisis hormonal me detectaron lo siguiente LH al limite osea 11,5 en un ratio de 2-12
FSH muy por encima de lo maximo 36 en un ratio de 1,5-12,6.
Prolactina 42 ratio 2-24
La TSH y T4 no mostraban anomalía.
La testosterona estaba teoricamente bien con 6 en un rango de 3-8. La RM no identifico problemas en la hipófisis

En el examen del urologo me detecto varicocele bilateral grado dos con atrofia testicular en mas pronunciada en uno de los testículos con disminución de tamaño. Hasta la fecha yo no había experimentado ningún síntoma (tengo 57 años y dos hijos) me receto TRT y dostinex 2 pastillas semanales. A los tres meses nueva analítica y visita al urologo. la prolactina estaba en 0,36 muy por debajo del mínimo del rango la LH en 6 (mitad de rango) la FSH en 12 (máximo rango) y la testosterona en 10,5 por encima de rango. Me toca la revisión con otro urologo el cual sin pensarselo me remite al Endocrino,pero con nueva analítica para lo cual debo dejar de tomar los medicamentos (dostinex y testosterona) esta da de nuevo fuera de rango el FSH 36 y LH 12,6 y sigue por debajo la prolactina 0,56 la testosterona en 5,6 y la T libre en 9 en un rango de 6-25 a la vista del resultado el endocrino me dice que la TRT no es indicada y me ha mandado a realizarme otras analiticas mas complejas que estoy esperando a realizar. Mi duda después de todo lo que he podido leer es ¿Que puede estar pasando si Tengo normal la testosterona pero elevadas la LH y la FSH ¿Hay hipogondalismo? ¿es primario? ¿es secundario? Por favor si alguien puede aclararme algo se lo agradezco profundamente.

I was also primary because of testicle injury. My LH and FSH was above range as you, I was able to produce six children, play sports and workout. GOMy main benefit that I received from TRT was elimination brain fog. I believe you're levels are low enough to qualify you for TRT, finding a qualified doctor can be the hardest part.

Gracias por su colaboracion, yo tambien soy muy deportista hago ciclismo en carretera, unos 20 mil km año. Nunca habia tenido problemas en mi capacidades sexuales, pero desde hace algo mas de un año, siento como un vacio, como si me estubiera vaciando por dentro...es una sensacion no un dolor. El problema ya no es el plano sexual, es mucho mas alla, es no reconocerse uno dentro de si mismo.

It's great that you exercise regularly and as you probably know it is really good for mental health. I really think you would benefit mentally from good proper levels of testosterone. I know it has helped me a lot both physically and mentally. Now that I have good levels of testosterone I do enjoy exercising even more. My next issue that I'm working on it's getting good thyroid levels because like many men with low testosterone I also have hypothyroidism.

No tienes hipogonadismo aun con FSH alto. Tu testosterona esta bien, asi que yo no empezaria con tratamiento. No se porque te receto testosterona.
La dosis de cabergolina debe ser una a la semana.

Te explicaron por que la prolactina la tenias alta (se que te dijeron que no tienes adenoma pituitaria)?

Si paras la testosterona de golpe vas a ser hipogonadal por varias semanas. Tu sistema se normalizara especialmente si la prolactina esta normal.

Estas tomando otras medicinas que te subieron la prolactina? Te hicieron el examen mas de una vez?

Te agradezco profundamente tu interés por que me consta que dominas el tema.
Efectivamente la RM no arroja ninguna patología y me consta que el "medico radiología" que la analizo tiene reconocido prestigo. El primer Urologo no sabia a que se debía por eso me prescribió la resonancia magnética. Una doctora internista con anterioridad al ver la cifra me indico que era por el efecto psicológico del pinchazo (sin ser entendido a mi me parece muy elevada para ese efecto) No me hicieron mas que un examen con la prolactina alta y si no lo indico yo en principio tampoco me lo hubieran hecho de la testosterona libre. He tomado por prescripción medico familia Urbasoline 500 mg durante el ultimo año, ya que en principio me detectaron la bilirrubina alta 2,06, después la doctora internista me dijo que tengo síndrome Gilbert. Dentro de mis características siempre he tenido los hematocritos altos en torno al 52-54 % con un volumen corpuslar alto de hecho me decían siempre que daría dopage en caso de inspección (bromas por mi afición al ciclismo). Otra característica es que siempre he tenido alto el hierro y ferratina si bien en el ultimo año debido a un cambio total en mi dieta estos tres factores los he regulado Bilirrubina actual 1 (referencia0,2-1,2) hierro 119 (referencia 40-150) lo he llegado a tener en 250 normalmente. El colesterol lo tengo en 208 actualmente
El urologo que me receto la testo y dostinex es cierto que me pareció que "se tiro al monte", pero no me sentí capacitado para discutirle.
He dejado totalmente el tratamiento y tienes toda la razón estoy en un estado que no es normal ni de lejos, incluso con una sensacion de "aturdimiento" como si no estuviera en mi. La endocrina me ha mandado un análisis "amplio" para la ultima semana de mayo, me gustaría y te agradecería que si puedes, cuando tenga los datos los incorporo a este hilo ya que como te decía, por lo que he leído de tus publicaciones, te considero una persona con conocimientos al respecto.
