If I did decide to go to defy medical for my symptoms does anybody have any idea of what they would do to treat me with my free test being low normal and my total testosterone being decent in the 500 to 700’s? I also have high normal shbg. Not sure what they would do for my low normal free test. Since I’m 26 I’m guessing they would choose another route besides TRT.

There are certain drugs that can be used to suppress SHBG serum concentration levels that will provide for higher Free Testosterone levels. It's not a common protocol as SHBG tends to find its own level in men and many Docs don't like to play with it; but it can be modulated if necessary. Talk to Dr. Saya and get his opinion that's your best option at this point.

There's a reason why some with high SHBG don't have as many symptoms as other guys, there's a variability in the efficiency of SHBG, too efficient and you have symptoms with high normal SHBG. Perhaps some day there will be a test for that as well.

Has anybody ever heard of Body Logic MD? Its located all throughout the United States and I live in St.Louis. I would love to find a private hormone doctor in this area and from what I have researched so far Body Logic Md is the best choice here. Anybody know of any good private hormone specialist in St.LOUIS area ? Or is Dr Saya my best bet?

what are your free t levels?
your total T fluctuate between 500's and 700's ??? Those are very wild swings. Ive never seen that unless you are on T replacement?

I think your best bet is still Defy. Only because it's easy for you being an American, you can get Labcorp labs easily and can do pretty much everything else over the phone with Defy. Why try an unknown variable when you see how Dr. Saya helps everyone else here... it's hard to ignore that. Hallendale would ship your Rx's directly to you. It's a no brainer for sure. Anything else you do has some risk.

I have had my testosterone tested several times. My lowest total testosterone reading was 505ng/dl I believe. My highest total testosterone reading out all my test I had got was measured at 730ng/dl. I don't know why my levels fluctuate like that and I am not on TRT. My free testosterone levels have always been in the low normal range. My most recent free T levels were measured at 77pg/ml with a range of (46-224)pg/ml. The other test measured my free T at 58.3pg/ml with a range of (46-224)pg/ml.

What about your thyroid? A poorly functioning thyroid can mimic low T symptoms.

My TSH and t4 were both great but my free T3 was barley below the range on my blood test. I was told by my primary that as long as my TSH is good Im fine.

TSH provides extremely limited information about the state of one's thyroid. An ft3 below reference range is a flashing light that should attended to.

I lived in a state of depression for most of my life because i was low in FT3. My TSH was perfect and thus I was declared normal. My HRT MD, expert.. put me cytomel or NDT immediately and got my FT3 range to the very top. My cognitive function improved and I felt so much better. I didn't know that I had been hypo because of low FT3. I think thyroid and trt are the most important hormones for the large part. Also with your thyroid optimised your Testosterone and free T may go up.
