Hallo Nelson!

New to trt and having problems and I think and trust that you are the man to ask when having trouble.

My trouble!

Man. 53 years old
On 250 mg test (Enanthate) a week (divided into 2x125 mg/week) for 10 weeks now.
Taking a mild AI (Triazole by DS Sports)
First: Felt awsome!
Sleep good
Sexdrive and libido like I was 18
Training and energy = Top
Then:
I go to sleep around 2200.
Wake up like 0300-0400 and can not go to sleep again.
Have a libido when I wake up but then there is no more libido that day even if my wife tries hard.
Penis size seems to have decreased.
My energy is good and training goes really good.
Size and strenght goes up, bodyfat down.

I then excluded the Triazole and after 5 days the libido and the sexdrive was on top. Still problem with the sleep.

Was then recomended to take 125 iu of hcg and did so for 7 days before I crashed.
No libido and terrible sleep.

So now Im back on only testo 250 mg/week and 15-20 mg melatonin to see what happends

So what do you think?
Estrogen? To low/high? Other endocrine problems?
But.. Were do I start?

You start with labs. Without them, all we're doing is guessing. Pull total and free testosterone, SHBG, estradiol/sensitive, DHT, DHEA, CBC, and CMP, TSH, t3, and t4. Post them along with your pre-treatment labs and we'll open a conversation on where things might be.

Some questions: Why such a large dose of testosterone? Your SHBG was high? You mentioned estradiol - is that why you're trying to manage it? Where was it sitting prior to the start of therapy? What other health challenges might you have? Medications or supplements that you take?

Waking up every night between 3-4 AM is often caused by a Cortisol spike. I know because I lived with that problem for years until I found ways to help my cortisol work more normally.

Cortisol should spike after you wake up in the morning, so the high point of the day should be around 8:00am (Plus or minus) When I was waking every night at 3:0 my cortisol was low all day and then spiking high at night, which was waking me and preventing me from getting back to sleep.

The way to test is the 4-tube cortisol test. You get 4 small tubes and you spit in them. When you first get up, around noon, around 6:00 and then around 11:00 or midnight.

Hate to hijack thread, but what do you do for your cortisol? I wake up every morning between 3-4 AM, and can't stay awake between 2-4 PM.

Agree that this is pointing more towards adrenal imbalance. Run the 4x saliva kit that CW has mentioned, and get it with a DHEA correlation analysis. Needing to see the circadian trend, total burden, and understand what stage/level of adrenal fatigue is present, IF indeed adrenal issues are present. Hate to hijack thread, but what do you do for your cortisol? I wake up every morning between 3-4 AM, and can't stay awake between 2-4 PM.

I found that if I take 25mg of micronized DHEA before bed it prevents a late night cortisol spike. Its not 100% perfect 100% of the time, but since I started that I now still sometimes wake around 3:00 but when I do I get back to sleep in less than 5 minutes where before, I would be wide awake until my alarm went off to get up and then I would be dead tired for the rest of the day. Some nights I do not wake at all until morning.
