At little background first. Im 35 years old and for the last 5 + years I've felt like crap. Trouble losing fat, Recovering, High Anxiety etc... Last year when my daughter was born and we hit our out of pocket max i had my PCP run all my labs last september and they came back as follows.

Test total 334
Test free 57.9
Test Bioavailable 131.6
SHBG 21
Album Serum 5
(no estrogen though, he did however run a cbc and thyroid panel which all were in normal range, can post if needed)

His response was your levels are fine. I disagreed and said maybe for a 70 year old and made an appointment. I tried to get him to prescribe Clomid from my research but he declined, then i mentioned hcg and he went for that. I tried 2 11,000 IU bottles at 500 every 3 days and saw zero improvement so I just stopped. Never got post labs though. Fast forward 3 months and I'm still not feeling right so i decided to get labs done from a trt specialist and I'm waiting for those to come back. My question is if they come back like i expect them to, I'm going to make an appointment which will be about a month out. I still have 11,000 iu's of hcg. Should i try to run that (at what dose?) for the next 3 weeks and then have labs done for just test and see where its at. My thought process is if i see zero increase in test then doesn't that mean that its my testicles not producing which would point towards my testicles being the problem not my hpta and maybe warrant skipping a Clomid restart and going directly to TRT. I figured best case scenario my level does go up and then my testes are primed to try a climid restart.

Did you ever have a test for LH? It seems doing either hcg or Clomid without knowing LH numbers is kind of pointless.
 
Since you've had more labs for this "specialist" I wouldn't do anything more at this point; HCG. BUt my friends are right, LH/FSH could be an indicator for being a Clomid restart candidate as those two tests should show if you're nuts are capable of making test.

Unfortunately no i don't think my PCP knows the first thing about hormones. I should have labs back by the end of the week and can post. What LH level would warrant this type of protocol and wouldn't if my LH came back decent and test low. trt would be the option. Also is there anything i should be doing in the meantime. I just don't really want to wait any longer to start feeling better if possible.
 
This is a long road no matter which path you take you should drop that start feeling better ASAP mentality because you'll just get frustrated, it's not instant results no matter what you do.

You really owe it to yourself to invest the time, and potentially the money, in working toward a successful protocol. There really are no shortcuts, but there are some things that can make the process far more difficult. If you try and self-direct your protocol - when you really don't understand the fundamentals of androgen management - you can wind up in a ditch. When you stick with a doctor who clearly knows next to nothing about the challenges of male hypogonadism because your insurance covers all or most of the costs - you can wind up in a ditch. When you accept a doctor's assurance that everything is "normal" because your test came back "in range" - you can wind up in a ditch. When you try and short circuit the process and make decisions without proper labwork - you can wind up in a ditch.

Many, perhaps most, of us have done one or more of those things, and made other mistakes as well. But success is possible. There are doctors who can help you. We can offer suggestions on how to find them.
 
There is no way to tell which you are with the labs you have.

Like most guys, their doctors only order the minimum of tests that they possibly can order. No E2, LH, FSH, prolactin, DHT or DHEA-S.
 
The best way to get moving toward success quick as you can is to ditch your GP/Endo/Uro and get with one of the big names, as it were...Dr Saya/Dr Crisler/Dr Shippen, et al.., most GPs and their contemporaries are not going to get the job done for you.

So got my labs in today since sept numbers have dropped futher

test went from 334 down to 285
test free from 57 to 9 ( i feel like this was a different test the quest did) number seemed to change drastically
SHBG from 21 - 35.6
Following tests were not run on initial labs in sept 16
Estradiol- 14.9
LH - 2.7
DHEA Sulfate- 471
TSH - 1.390
PSA - 0.6
Anion gap- 24

Can you provide reference ranges, please? Thanks. Different labs establish different ranges and it helps us engage with you if we understand where your particular value falls.
