I am in Spain, 50 years old. I've been having low T symptoms for a few years, but I was able to cope with it because it was only a "performance drop", until a couple of years ago, when severe erectile dysfunction appeared and I could no longer have an acceptable sex life, in addition to a serious affectation on my physical well-being and mental performance.
I had lab tests done and total testosterone was at 369 ng / dl, at that time no free T was determined. Everything else normal. May be that T level was not drastically bad, but the blood sample was taken in one of those few "better days" I used to have
I started with T Cypionate, then had symptoms of E2 elevation and added anastrozole, from there I had a great improvement in all areas, although I often keep needing the help of at least 2.5 mg of cialis which makes everything perfect.
The weak point of my protocol is the absence of HCG, which is not available in Spain and has been the cause of irregularity in my treatment due to I can't psicologically deal with the shrinkage of my testicles, which has led me to try to get off TRT few times, but it doesn't matter if I use tamoxifen or clomiphene for recovery, my testicles go back to almost their original size and hang again but the symptoms of low T return overwhelmingly and make me remember again why I need to be on trt.
Now I am forced to start up again with T cypionate after 3 month of unsuccesfully coming off..
This time I am considering to add HMG lepori (75 mg FSH + 75 mg LH /ml) which do is available in Spain.
I am not interested in fertility ( I already had children) but hope it would allow me to "preserve my anatomy" and be finally happy with my trt (if I can continually afford to buy HMG).
Does anybody has any experiance with HMG while on trt (without HCG)?
Any advice on this? What would be the right dose and frecuency?

I have ordered some medication from Canada some time ago and it was delivered to me in Spain without problems. But the main issue ordering HCG from abroad is the cold chain that should be kept during transportation. I do not know either if the importation of medicines from the USA will be penalized or prevented by customs in Spain.
 
I live in Portugal and I can buy Ovitrellle at farmacies. Pregnyl has been taken out of the market for sometime.
It should be on the market in Spain as well if I´m not wrong.

Thank you Tommy. Yes I didn't look at it before, but now I just had a look in Vademecum.es and it is available in Spain, but in a preloaded syringe of 0.5ml with 250 mcg, single dose for a price of 50,63 €. So I don't see it can be usefull and affordable for TRT.
Can you give me more info on how it is presented in Portugal and how you use it?

Now I have found out that 250 mcg of ovitrelle is as strong as 5000 UI of HCG, but it comes in a single dose preloaded syringe of 0,5 ml.
How to take 25 - 50 subdoses out of it?
I think HCG is out of the market in all Europe, solving this problem with ovitrelle wuold b wonderfull

The HCG comes in powder and when you're ready start using you mix it with bacteriostatic water. After that you just keep the vial in the refrigerator.

If cost is not a problem for you then yes HMG can replace HCG. HMG has LH and FSH so you are replacing the exact hormones that got shutdown by TRT.

Cost do is a problem... 10 doses (liquid + powder) of 75 UI cost about 150€. Somebody told me it is possible to extend those doses with good results for my purpose through a period of time of 3 months, which then could be affordable, but the manuacturer says that any remaning amount after mixing should be inmediately discarded. Then I don't know how to extend it, cause it has a short half life so probably it is not good idea to administer 75 UI (full dose) weekly or longer...

I think to avoid testicular atrophy half dose E3.5D might work. The problem is half life of LH and FSH is much shorter than HCG. So 10 vials would last you 10 weeks

I am happy to know that I don't need to leave my testes behind while on TRT... Now I see I have two options here in europe to face the lack of HCG.
Now I am figuring out which one I will take since both are tricky procedures for dosing and administration. (and one is more expensive than the other too ;) )

What if buy 0.3 ml insulin syringes and pre-fill the three (or two) bars in each one from the first day, then there is only one time of manipulation, less risk of contamination and you are ready to go immediately for every dose?
You can get insulin syringes with incrustrated needle, with no dead space, and they come with a cap to guarantee sterility, which you can replace for protection after pre-filling.

How often do you take your "3 bars" dose?
 
Sounds good however I -prefill everything into one syring that I keep in the fridge with inside a plastic bag with alcohol etc.... and when time to inject I just draw 3 bars. But If you can get 0.6ml syringes that sounds even better. Keep us informed how it goes. In Portugal it cost around 35€ per pre-filled syringe but taking into account that you only need 2x0,3ml per week its not that expensive.

Here in Spain Ovitrell cost 50.63 €, but considering that it can be extended up to three months, it is still no expensive...

It sounds good, but I am still concerned that it here comes as ampuoles, not vials, with the 75 IU each FSH & LH, and the manufacturer says: "Each ampoule is for single use and the injection should be administered immediately after reconstitution". I am afraid it could be because it loose its properties very fast after reconstitution.
I'm talking about the brand "IMG Lepori". There are other brands which like "Menopur" that allows multiple dosing, but are much more expensive, and I descarded them.
