Good Evening,

My first post to the forum looking to get some opinions on my recent blood work.

I've been on TRT for about 10 years. For the first 5 or so, things were great on 150-200mg Cypionate per week, nothing else. About 5 years ago I began struggling with libido issues and some ED. Over the last 5 years I've experimented quite a bit with dosage, injection frequency etc. Recently my TRT clinic suggested I try Transscrotal cream due to the increased libido. I started on the cream about 2 month ago and overall its been really great. Improved mood, energy, motivation, libido. Felt the best I have in years.

The one area that has gotten worse is the ED. Its worst Ive ever experienced where cialis wont even phase it. I just got some labs back and curious if anything jumps out as the culprit? Clearly the testosterone is quite high, but I've had these levels before from injections without such severe ED.

Dosage: 200mg cream, 4 clicks daily to scrotum, 2 in am, 2 in PM.
43, non-smoker, non-drinker, healthy diet, no rec drugs.

These labs were drawn 9 hours after application, 3 hours prior to next application.

Thanks

Your total, free and DHT are sky high. And that's a good amount of time after your last application. What were your numbers on cypionate? I'd definitely cut back on the cream applications. Maybe try 1 click in the a.m. and p.m. and see where that gets you. Aim for high normal. Your estrogen really isn't that high in comparison, which may be causing a ratio of testosterone to estrogen that is causing you problems. Not sure if HCG would do anything for you, it's geared more toward libido issues.

Yeah i also support what TLawyer above mentioned re going lower and seeing what happens. Have you been on these same/similar sky high levels all the 10 years uve been on TRT? If so I wonder how other markers of your blood looked over this period and what side effects did you battle along the way. Context is everything and less is more, sometimes is the way to go.

I had this problem too on creams until I switched to once per day application.

Thanks for the feedback. Yes I was quite shocked how high the cream got my level, especially right before my next application.

Do you suggest getting any additional blood markers with my next round of tests? I tried to gather recommendations from the forum and used Discount labs.

Yes but when I switched to just morning application on scrotum it fixed the issue.
There is a study that Madman posted which shows that one click should give you good levels for 24 hours. Defy also recommended once per day application in the morning if I wanted to use it. I don’t think the twice per day would be necessary in most cases.

For me applying at night not only caused me ED but it disrupted my sleep. Once I switched to just morning application it fixed everything. I like trying different things. My next experiment is going to be jatenzo.

Thanks for the detail. I like the ability to adjust w daily granularity. I Just switched to 15mg Prop per day. I will get bloods in about 3 weeks or so.

Do I have this right. You ditched the cream completely and now are doing 15mg/day of test propionate?

One thing that I found too high a dose of injectable Testosterone caused me to have worsened ED to the point that I had to wait until my levels went down. It is useful to have regular bloods done to check over the week how your levels fluctuate so you don't have too much in your system - too much really does not solve anything - it just makes things worse - well in my case that happened.

Thanks for feedback. Thats my goal this time, get lowest dose that resolves issues. I think when I was younger my body could overcome higher levels and such. The older I get I need to turn down the octane a bit. :)

Correct Stopped the cream all together. The severe ED scared me. I am on 15mg prog a day. Its been 7 days so far since switch. I still feel good, libido has dropped a bit but ED is improving.
