I'm a new member looking for advice. I got married almost 6 years ago and I've had zero libido from day one. I had been having issues since after turning 40 (I'll be 52 in April). Issues like fatigue, no motivation, low libido, etc. I had not been sexually active since my early 30's and was not sexually active while dating my now wife. But while dating I DID have desire - but literally zero since married. Sounds bad I know. We go months without having sex. So a year into the marriage I went to my doctor. My total test was around 500 so of course he did nothing. Every time I see him I mention it but he has no advice. I decided to go to a men's clinic and here are some of my numbers: Total test = 383 / Free test 4.5 / Estrogen 16.6 / PSA .7 SBGH 73 / and was told my albumen was normal. I'm not sure what all this actually means but I've decided to start injections. They want me to do twice a week (can't remember how much - maybe 50mg x 2). they also want me to take Adrenal for the first two months and Boron. And all of this is quite expensive (around $160/month). My question is, based upon my numbers, am I heading in the right direction? Also, is what they are prescribing proper for TRT? Thank you all in advance. I'm very happy there are sites like this to help with this issue. One more thing, as I said, I'm almost 52. I'm 6' around 185lbs and have been fit all my life. I'm very healthy with no history of disease. I'm on no medications except Ranitadine for acid reflux as needed. I'm a former long distance runner. Now I mostly lift weights to stay in shape...

Well, with high SHBG and low free T, your symptoms can certainly be stemming from hormonal issues. And 100mg of test is often a good starting dose. That being said, it sounds like there maybe some relationship and psychological issues in play as well. Do you get turned on by things other than your wife (porn, fantasies)? Do you masturbate at all? Any morning/nocturnal erections? Frequency and quality of erections can directly affect libido (especially in our age range over 50).

Yes, I do believe there are some underlying psychological issues. And, although I don't do the porn thing, I do get turned on thinking about other things/women - although I work hard to avoid that. Masturbation is extremely rare - I feel guilty because I feel that should be reserved for my wife. So maybe 3 or 4 times a year. Nocturnal emissions - maybe twice a year. Morning erections - maybe once or twice a month but even when they come, there is no real desire for sex.

are you diabetic or any have any thyroid issue? what is your Prolactin level? You should get morning erection almost daily. Plus I think you have lack of physical attraction toward your wife also.

Your SHBG is very high and definitely why Free T is incredibly low, sadly there isn't much you can do about it expect TRT.

It seems a lot of doctors who should be knowledgeable in this area of medicine most often are little to no help at all. Many healthcare providers are hesitant to initiate men on TRT due to the belief these treatments increase the risk of cardiovascular events.

Yes, I do believe there are some underlying psychological issues.

Low-T can have a profound effect on a man's psychological makeup, a man needs adequate testosterone to act like a man.

Thanks for the honest response. Maybe Mountain Man will chime in as he is therapist.

I don't want to steer you to porn or have you do things that run counter to your beliefs, but you will need to take action both hormonally and with your penis to keep things going as your age. That includes more frequent erections and orgasms, even if self produced. Better yet with your wife, even if you don't have intercourse. There is a direct feedback loop with all of this. Just something to consider...

From my experience, if I am not achieving daily erections 5-6 days a week and having 3-5 orgasms per week, my sex drive and erections drop off even on TRT. Actual sex with the wife 1-2 times per week as our schedule allow.

I have no thyroid issues and am not diabetic. I had bloodwork done at my PCP on Wednesday prior to starting TRT. I’ll check to see if prolactin is part of the blood work. My wife is often stressed out but other than that, I’m very attracted to her. She’s pretty hot - and 10 years younger than me.

Good to know. Yes make sure your PCP does a Prolactin level check. High prolactin causes ED issue, plus causes low-T also. Feel good hormone Dopanine also correlated to Prolactin level.
