Hi All...

I have been on TRT for almost 2 years. I have everything mostly regulated. I am 38 years old.

I do daily injections of .13ML and my SHBG is generally 13-15. I also take a daily cialis from empower 7mg. I am on an antidepressant Viibryd which is not supposed to have sexual side effects and my effects had started before this.

I have sleep apnea

My testosterone levels are normally run in the mid 600's to the low 800's.

My hematocrit jumps up quickly so i need to donate blood doing double red every other month. Right now it is 54.5 and i am due for it.

I smoke cigarettes. I know i need to quit. I just had a stress test and everything is normal so no known heart problems.

My Estradiol currently is 29.3 and ultrasensitive was used. I take anastrazole as needed. Usually .25 at a time.

My DHEA-S is 359

My Thyroid is normal.

My A1C is 5.7 which i would love to be lower. I am 6'2 and weigh 218 and go to the gym a few times per week. I am in fairly decent shape. I am fit.

The problem i have is i can get erections but my penis many times has no sensitivity and i have a hard time ejaculating. I try to avoid porn and do masturbate on occasion. I also use Man 1 oil every day.

Any thoughts to what i could be looking at to solve this? I am too young to deal with this shit especially since i am going through a divorce and having a fun sex life would be nice right now.

I´m soon 58 and after starting TRT I noticed after sometime that my penile sensitivity became less than before. I have no doubt about that it´s a side effect.

I just thought I would throw this out there. My buddy loses his penis sensitivity when he uses Viagra or Cialis. Without the ED meds his sensitivity is fine.
 
Ive heard and/or thought I experienced the sensitivity thing with Cialis. I by default use Viagra now.
Besides that, remember when you look at that E2 reading, 29.3, that's your LOW point. We know E follows T, so E will rise with your injection. I would look, given your SHBG of ~15, to have your E2 LC/MS/MS mirror that...15.

This works fairly well, for me. My SHBG is about the same and having my trough E2 in the single digits resolvs a lot my problems, not all, but a lot.

I have low SHBG as well and I lose erections and sensitivity if estrogen gets too high, so an E2 of 29 could be high for you given your low SHBG and one needs to also consider Free E2.

The reference ranges for TSH aren't normal, so your levels may not be normal for you.

Before TRT I had virutally no sensitivity at all. My E2 was in the toilet. TRT has provided *some*sensitivty. It seems best when I am between 30 and 40. I did lose sensitivity and ED was worse when my E2 got near 70. Once it came down everything improved. This happened via lowering of E2 by reducing HCG from 250 twice per week to 200 twice per week but my T also dropped. I am now on more T Cyp and am hoping that my E2 does not get too high or else I may have to start on an amortase inhibitor.

I have also found that Viagra helps with climaxing for me. If I don't use it I am not hard enough to get stimulated. I can still climax but it is much much more difficult simply because all of the right parts are not being stimulated.

BTW, ditch the porn 100% Its bad for you REALLY bad. Also, I may be in the minotriy here but masterbation is not really good for your penis. It can contribute to sensitivity problems. The vaginia was designed to make the penis feel amazing. Our hand is NOT a vagina and can lead to becoming less sensitive and only respond to that sensation.

Not popular to say but I couldn't agree more. Lowering my E2 to match my SHBG has been a game changer.

I took Nelson's advice a couple years ago and cut back on the porn. It's made a difference in both my libido and quality of erections. There's a heavy poster here who would get pissed at the notion that porn could possibly affect libido/erections, proudly boasting that he watches a ton of porn...while educating us on the army of supplements he takes to help his libido/erection. Always thought that was ridiculous.

Exercise stress tests are at best 70% predictive. Nuc stress a little better. You have no idea how much occult CAD is out there. It remains the #1 cause of death for both men and women. The impact that the cigs have on your vascular endothelium cannot be minimized. Stopping tobacco is the best thing you can do for yourself brother.

The point at which significant risk from CAD has also been lowered. Previously coronary artery stenosis of 50% was the cut off for “NORMAL” i.e. non obstructive heart disease. Yes, if one of your coronary arteries is narrowed by 50%, it is considered “normal”. However, the risk of premature death in individuals with less than 50% stenosis has been reconsidered to be higher than thought.

Generic Zyban works so well to quit smoking and it is much cheaper than Chantix. You can get it for like 20 bucks per month cash. All you need is 2 months worth or less. Problem is the Zyban is an anti-depressant that reduces penis sensitivity while you’re on it. HCG seems to always boost my libido and sensitivity a fair amount.

What is your free testosterone level? I bet it’s high. If your HCT keeps rising so much, you need to lower your dose. How many times have you donated blood recently? If you’ve been doing that for a while, guarantee you tanked your iron levels, which absolutely can effect sexual function. You should only be donating a few times per year. Especially if you are doing double red each time.

Please post your thyroid labs. Saying that they’re normal means literally nothing. Especially if it was a doctor that told you they were normal. You most likely also haven’t had a full thyroid panel done. Only having a few thyroid labs done is almost the same thing as having no thyroid labs done. There’s T3 receptors all over the penis. Not having proper thyroid levels can absolutely effect penis sensitivity, and overall sexual function.

Definitely need to get your A1C down. That’s an easy fix though. Increase the amount of weight lifting sessions you do per week, cut out dairy, cut out gluten, lower carbs per day, replace those calories with protein and healthy fats. Cut out added sugar, and cut out artificial sugars. There’s other things you can do, but those are the main things, and should absolutely do the trick, especially considering you’re A1C wasn’t even that high. Just want to get it down closer to 5. Insulin insensitivity absolutely plays a role in sexual function.
