Just started Trt, having second thoughts

Hi all.
Forgive me please if my post is long and incoherent, I am typing as I go, trying to summarize everything with a single question.

I think I always had low/ish T. My right testicle was in my abdomen when I was little, never really trully developed, is about 2,5 cm long only, still migrates to the groin sometimes. Left, I had varicocoele, probably untreated for 15 years. Just got my second kid and finally decided to check what is wrong. (Still being fertile I hold for a miracle). Was supposed to be the prostate or low T in my opinion. PSA 0,69; TT 3,53 and 3,08(3,5-11,5).

I live in a country where urologists know about andrology and hypogonadism only by name.... I myself am MD, but my area has no touching points with andrology whatsoever, so I am here. Changing doctors would not help, they would have to do what insurance allows. With 3,08, all I got is was: "everything normal".

Well, it is not. I am only 39 yo. I haven't had sex without cialis for the last 10 plus years. I got some abdominal fat. I sleep bad. I am always tired. Brain fog is terrible. I always thought I was a lazy person, but I am just unmotivated, constantly. I have problems with nothing, except with 0 libido and brain fog. I want to "want to" have sex, get spontaneous erections(I don't even know what that is any more)... Urology office was the only place I could have my blood tested. Next to 2 above, my SHBG was 29, my E2 14. Urologist would give me private prescriptions, but I would have to pay myself. Because I am 0,08 above. So he is useless. Being a doctor too, I could buy whatever I need and I would probably able to bear a cost.

After reading for tens of hours, started 2 days ago, with test E sc, 125mg/week(2 x 62.5) and 2 x 250 IU Hcg. With intention of testing my blood for a lot more parameters in 8 weeks. Urologist would test my blood regularely, and take me "as a Patient" since I am a 'colleague'. But yesterday I read these forums up and down and found some interesting cialis threads.

Now, I am an a**hole... I scr*w around. Sex with my wife is just a chore and impossible without cialis, if then at all too. Lasts until she is over and than I just want to be left alone for like a month. I look good, been lifting weights natty for 15 plus years. Other girls fulfill all my fantasies but when we are over, I don't think about, I don't fantasize about what happened for long really.

I am what you would call a slow metabolizer. 5mg of cialis only, I can go for 3-4 days, 10 times a day, outside of marriage bed. And a feeling of having more blood under actually does a little something for my libido, as in I AM actualy looking forward to the next encounter. I am yet to find a single data against daily cialis. I do not look forward to being married to a needle for the rest of my life just yet. If I had to choose one symptom reversed, it would be being ready to have sex 24/7.

The question is, what would you do if you were me, two options:
1. Continue TRT
2. 5mg cialis eod for a couple of years at least, first. From all I read, it doesn't lose effectiveness and sometimes even reverses ED.

Welcome to Excelmale. You could try Enclomiphene, it's possible you could have success with it. It may be something that you would like to research and see what results you will get. If then you are still not satisfied, you could consider trt again.

There are some suggestions of testicular insufficiency, in which case a SERM like enclomiphene might not be very effective. Have LH and FSH been tested? If these are towards the high side then this would at least give a hint at the form of hypogonadism.

I think in this situation, having doubts, I would opt for more regular use of Cialis first, deferring TRT until I was more sure about it. TRT is a greater commitment, and once the HPTA is suppressed it takes time to get it going again.

Vince, I just spent 10 minutes googling, it is not yet approved where I am, or at least I am to dumb to find it. I would have to order from abroad and hope it goes through customs.

Cataceous, no, LH and FSH weren't tested. This doc thought he was doing me a favor by letting me not pay for the lab tests, as I am his 'colleague ', but by doing so he didn't test all I wanted. Only what I listed above , thyroid and cbc. All good except TT imo. Since I obviously have 'testicle damage', I am leaning towards the conclusion that I have primary hypogonadism.

I work a hard job but I am still top fit. I bodybuild, I have a good amount of muscle, no heart disease, I could run for miles with no problems. Funny things are, my ejaculate volume was never reduced. I am fertile. Doc insists normal range is 3-10 ng/dL and I am above the lower line. Insurance can't pay since I don't qualify, and if they would it would have to be Nebido or gel. Which I would wanna avoid at all cost. Looking at primarily HCG prices, and test E, money does sort of play a role too. Compared to a year supply of tadalafil, which would be around 120 US$.

Let me directly ask 2 more dumb questions:

1. Is it possible that cialis can work without loosing potency for like 5 or even 10 more years? Difference being, this time "scheduled regular" use.

2. If I choose the cialis option, I would simply ignore having injected test e and hcg once? Nothing further to do regarding one injection?

Would like to make a decision before Thursday morning, when the next injection is scheduled, with the help from all of you... What would you do in my place....

I've been a regular user of Cialis for over 5 years now. It does not seem to have lost any of its potency. If you injected just once, it won't hurt you. I would just stop trt now. The odds are trt will make you infertile, but one injection won't hurt you.

I am a great responder to Cialis, I hope I will be an even better one with regular therapy. Acoording to some studies, it can even increase Testosterone a little bit :). Giving it a try, hope it will benefit me for years to come, before testing my hormones again.
Trt always remains a possible option, but I am disposing of syringes and needles ftm, as we speak.
