Obese male, age 43. Finally decided I needed to make some changes in my life.
Went to a test clinic and original test results before therapy began:

Measurement

Range

DHEA-S

151.5ug/dL

102->416

Testosterone, Serum

291

LH

4.7

1.7-8.6

Estradiol, Sensitive

18.1

8-35

SHBG

27

16.5-55.9

I was put on a plan that included:
50mg Test Cyp 3x/week (total 150/week)
350iu HCG 3x/week
.125mg anastrazole 3x/week

Just got my bloodwork back after 2 months on this and received some crazy numbers.

Testosterone, Total

1202 (HIGH)

Testosterone, Free

268 (HIGH)

range: 35-155pg/mL

SHBG

26

DHEA-S

126

Estradiol, Sensitive

32 (HIGH)

range: <29

This was all measured on the trough, so right before my shots and pill.
I am thinking i should lower my Testosterone portion but leave the HCG and AI unchanged. But the Free Testosterone is alarmingly high, and I'm not sure why? Unfortunately that doesn't seem to have been measured on the orig bloodwork for some reason.

I used a free T calculator online and came up with this if it helps since no Free-T on the orig set of bloodwork:
PRE-TRT:
Free Testosterone 6.25 ng/dL = 2.15 %
Bioavailable Test 153 ng/dL = 52.6 %

POST-TRT:
Free Testosterone 34.2 ng/dL = 2.84 %
Bioavailable Test 802 ng/dL = 66.7 %

Anyone have suggestions for me? I don't really feel like I've benefitted all that much. Def not lost much weight.

Two months into TRT is still very early in treatment and it could takes months to dial in your TRT protocol. HCG can negatively affect men and make it feel like TRT isn't working and isn't for everyone.

If you friend has a thyroid problem, TRT can't work. I would tell your friend to at least have a thyroid panel done checking the TSH, Free T4, Free T3, Revere T3 and antibodies if he is still struggling after he has had a chance to dial in his TRT protocol.

My two cents is. Cut the testosterone dose to 50 mg twice a week and eliminate the AI. Personally I like HCG, so I would keep it. Retest after 6 to 7 weeks. If needed, I would adjust from there.

I have some of the thyroid numbers.

TSH = 1.82 range: 0.4-4.5
Free T3 = 3.3 range 2.3-4.2

that looks like all they ran of thyroid numbers, but doesn't seem too awful.
