Female Target Hormone Values - Getting Started with HRT/Thyroid

Hey guys,

I've been on TRT and feel like its really made some changes in my life. Well, my wife (fit 39 yrs) noticing symptoms of her own is interested in looking into HRT/Thyroid for herself. She got a blood test and will be going in for her first visit. After looking at her results I am realizing I am clueless about female target values for Testosterone, Progesterone, Estrogen, and Thyroid (T3?). I know target values are largely individual but are their female targets that can give us a ball-park? I am looking for some insight to my wife's initial blood test prior to her first visit. I'll post the results below (caveat: I realize the panel she got doesn't include everything she needs).

Your wife's testosterone clearly is a problem, I'm not sure the relevance of high LH, but if it was a guy I would say he is primary hypogonadal. I've picked up a few things on Dr. Saya posts and I can tell you dosing wise 20mg weekly would put most women in the supraphysiological range, 10-12mg split up multiple times a week is best for women. Most women in their 40's could use a little progesterone, by the time women hit 40 they just don't have the hormones they did in their 20's. Women hormones drop off a cliff long before a man's does.

It would be a good idea to run a thyroid panel checking Free T3, Free T4, Reverse T3 and antibodies.

High normal is usually targeted when SHBG is midrange or higher, lower SHBG and it's better to target mid range levels do to free testosterone being high when SHBG is lower. You might need to test SHBG before your wife goes on TRT, you need it to design a protocol. Of course if you go with Defy it's like a self driving car, doctor takes care of everything.

What about for estrogen, & Prog? Shoot for high-normal or low-normal? T3 I am guessing we want high-normal?

At a minimum you want 40 to 50 pg/mL to prevent bone loss, but 60 pg/mL or higher is optimal, levels of about 100 pg/mL for younger women. Free T3 is better midrange or higher. Progesterone I don't know, progesterone is a precursor to the sex hormones estrogen and testosterone.

Females hormones are always changing, men's hormones are more stable. Now you know why...
