Sorry guys I hit the send key by mistake on the first post. Tri-Mix will turn you into a 20 yr old. I did get carried away on my third injection and had a 3 hour erection but learned my lesson.

Baught, I could not do the 3hours often but That could have been good lesson. There are some wonderful thing that can be accomplished in 3hours given a few recoupe or as she would call them cuddle breaks. We are looking forward to being able to be more spontaneous with the confidence that everything is strong.

Hello, Jay in Columbus. I also live in Columbus, Ohio. I've recently begun to see Dr. Gregory Lowe @ OSU urology. I'd prefer Cialis, but insurance refuses to pay, so Dr Lowe suggested Trimix. I'll likely be getting Rx filled this week. Dr. Lowe is good.

Jay - Here's a good tip for you; do a test dose on yourself first. Do the dose as prescribed, don't do less or more.

Get used to the injection procedure. Get used to the erection. Have fun.

This way, the next time you are ready to make love to your wife you will know what to expect and be more comfortable as well.
 
Gene is spot on with this.......I made the mistake of under dosing and trying it with the pressure of performance immediately......not a combo for success. 2 years later its now a cake walk but with anything first time its stressful.

Gene is telling it as it should be......

Congrats, Jay! Dr. Lowe referred me to Crosby Drugs for Trimix. For many reasons, I didn't get there to pay for it in a timely manner & they returned my rx to stock. I called yesterday to finally get it filled & Crosby pharmacy told me it may be a business week until it is ready again. Ugh. The technician did tell me that if rx is ready sooner, they'll call me. I'm requesting a lesser quantity to be dispensed as my first fill. In the office, Dr. Lowe injected 0.2 mL(=20 units) & my resulting erection lasted too long(tough to imagine, I know). Dr. Lowe advised me use a lower dose next time & titrate up as needed. I can tell you that the erection quality was unlike any I've had since my teens: Rock solid!!

VERY convenient for me. I live right up the street from Crosby's. I went in today thinking I would pick up the script, but was told it takes 3-5 days to compound it. I thought that's what the Doc told me, but more likely, it was what I wanted to hear. NBD really. I'm just glad to finally be on track!

Well, I finally got the Trimix #6. The formulation for that is 29.4mg/1mg/10mcg (Pap/ Phen/ PGE). The scrip dosage is .35, but I did a cautious .20. It had about the same results as at the Doc's office- maybe a 60% chub that went away quickly. I'll likely try the prescribed amount next time and maybe go from there until I find what works best.
I've seen formulations all over the place. Mine, I notice appears low on the PGE. Is PGE the component that creates the wood?

TriMix: Papaverine,Phentolamine, Alprostadil

TriMix is a compounded medication the combine’s papaverine, phentolamine and alprostadil in one vial to achieve maximum efficacy, lower incidence of pain,and lower cost per dose. Studies have been done that have shown the efficacy of all three agents combined into one formulation. Combination therapy was first introduced in 1991 by Bennett and his colleagues who demonstrated a success rate of TriMix of 92% in 116 patients. TriMix is often reserved for patients who fail alprostadil (PGE-1), fail Bi-Mix or for patients with severe penile pain from prostaglandin E1. Since TriMix uses lower doses of alprostadil,penile pain often subsides.

- Usual prescribed strength: Papaverine 18-25mg + Phentolamine 1.0-2.0mg +Prostaglandin E1 10-25mcg/ml



Alprostodil

- Commercially available for monotherapy as Caverject® and Edex®
- Mechanism: a prostaglandin E1 that stimulates the relaxation of the smooth muscle and dilates arteries in the penis increasing blood flow
- Studies have shown efficacy in 75% of men when injected into the penis
- Usual dose: start at 10-15 mcg inpatients who have failed oral therapy or 2.5-5 mcg in patients who have neurogenic or psychogenic ED and have failed or declined oral therapy
- Median dose is 12-15 mcg. Benefit is not seen at doses greater than 40 mcg. It is recommended to try a drug combination that includes papaverine, phentolamine or both.

- Side Effects

o Priapism: prolonged erection lasting more than 4 hours- must seek medical attention
o Penile pain- during or immediately after injection, occurs in upto 31%, may be reduced if mixed with other agents
o Bruising at injection site- can be minimized with a 30G needle,should be minimal with proper injection technique
o Hypotension- occurs when drug leaks out of injected area intogeneral circulation. Rare with alprostadil.
o Fibrosis- scaring of penile tissue is lower with alprostadil, mayresult after long term use of injection therapy
o Occasional increase in liver function tests

Phentolamine

- Commercially available as Regitine®
- Mechanism: alpha-adrenergic antagonist that produces a direct vasodilation in the arteries increasing blood flow to thepenis.
- Usual Dose: 1 to 2 mg

- Side Effects

o Painful prolonged erection lasting more than 4 hours
o Fibrosis- penile scaring
o Systemic hypotension (low blood pressure)
o Reflex tachycardia (increase in heart rate)
o Nasal congestion
o Gastrointestinal upset

Papaverine

- Not commercially available and must be used with phentolamine to produce hard erections.
- Mechanism: Phosphodiesterase inhibitor that results in smooth muscle relaxation allowing for an increase in blood flow to the penis. It’s used mainly in combination with alprostadil and/or phentolamine. Although it has been used since the early 1980’s, it is not approved for ED.
- Usual dose ranges from 5 to 30 mg

- Side effects

o Priapism- high rate of 1-6%
o Fibrosis- highest rate and can present as a lump within the penis
o Hypotension

I warned about not doing the prescribed amount and you saw the results.

Do as the Doctor prescribed if you want the results. You can always work down but you may be very disappointed if you go with a lower dose with your partner and you lose it sooner then expected.

Papaverine and phentolamine cause the dilatation of the corpus cavernosum and PGE causes the the venous blood flow to stop (that causes the hardness).

Certainly no harm- no foul in going lower than prescribed. I have read about several experiences where the prescribed amount was too much. Just erring on the safe side.
